Community Support Programs

Case Management Services

Our professional clinical staff provide assessment; individual, group, and family counseling; case management services; vocational services; housing assistance; medication evaluation; prescription monitoring as well as information and referrals for adults (ages 18 years and older) diagnosed with a severe and persistent mental illness and who live in Chittenden County. Our primary office is located at 300 Flynn Avenue in Burlington although many services are provided to clients in the community and at their homes. We also provide services to the homeless mentally ill through our site at the Homeless Health Project at the Safe Harbor Clinic, 184 South Winooski Avenue in Burlington. Treatment planning is tailored to individual needs. Clients participate in establishing treatment goals and in assessing outcomes. Length and modality of treatment are based on individual client needs. Our hours of service are Monday through Friday from 8:30 a.m. to 5:00 p.m. with after-hours crisis coverage provided by the Mobile Crisis Team.

Adults (ages 18 years and older) in Chittenden County who have been diagnosed with a major mental illness are eligible for these services, as are individuals diagnosed with severe personality disorders. Co-Occurring Disorders Treatment Program

The Co-Occurring Disorders Treatment Program (CODTP) is an outpatient program designed to provide fully integrated treatment and continuity of care and caregivers through time and setting to a vulnerable population with persistent mental illness, substance use disorders, and past or present correctional involvement, who are at high risk for relapse, for recurrence of symptoms of mental illness, and for re-arrest and incarceration. At the core of CODTP is assertive case management offered by a multidisciplinary team. The team provides active and ongoing treatment with a primary goal of reducing the emphasis from intensive institutional care/supervision to less intensive community-based services. The program is designed to engage consumers and accommodate various levels of motivation, compliance, severity, disability, and treatment readiness. For more information, please call 802.488.6006.

Residential and Community Housing Programs

Community Support Programs (CSP) Residential and Community Housing Programs serve adults living in Chittenden County who are considered to be mentally ill. An individual must be served by CSP to be eligible for housing consideration. A wide range of programs with varied levels of supervision and staffing is available dependent upon an individual’s needs. There are seven residential houses, three community housing sites, and a number of apartments. Some of the programs have staff 24/7, and others have staff support on a part-time basis. We provide case management services, supportive counseling, and activities of daily living education. Referrals from hospitals, community mental health centers, social service agencies, individuals and family members in the community, and case managers are made directly to the CSP Residential and Community Housing Director. Westview Employment Services

Westview Employment Services is a vocational program intervention available to adults (ages 18 years and older) in Chittenden County who have been diagnosed with a major mental illness and have a desire to pursue employment. Individuals may have a co-existing substance abuse diagnosis and/or legal issues. The Community Support Program and the Division of Vocational Rehabilitation are our primary referral sources. Services are provided at various sites including many locations at HowardCenter and in the community. Hours of the program are from 8:30 a.m. until 4:00 p.m. Some flexibility is available depending on the needs of the consumer for job-related supports. Employment counselors are available to work with consumers after normal business hours if necessary and requested by the consumer. Westview House

Westview House is a psychosocial, rehabilitation, recovery clubhouse located in Burlington serving adults (ages 18 years and older) in Chittenden County who have been diagnosed with a major mental illness. It is designed to offer a supportive environment and opportunities for self-help rehabilitation to individuals in the community who are recovering from psychiatric illness. Participants should have a history of major mental illness and/or co-occurring disorders. Westview House is open Monday through Friday from 8: 30 a.m. to 4:00 p.m., as well as on weekends and holidays and during the evening for scheduled events. Clients are referred to Westview by a HowardCenter case manager. Weekly Orientation is held every Monday and also offered in an ad hoc fashion to promote greater accessibility and ease of entry into the program during the Monday through Friday program time, if the coordinator and Westview members are available.
